Roadmap is the Explore phase of the Foundations for Greater Independence Program!
It supports your client to create a Roadmap towards a more engaged, connected and independent life, and provides them the Foosteps required to follow it.

As Foundations: Roadmap™ is delivered by the client's existing Support Worker during their already scheduled (and therefore already funded) support sessions, the additional cost of the program is very low, given its value.
Including all ten modules, the Roadmap Report & Footsteps Guide, as well as the counsellor-mediated progress review and planning session at completion, the cost averages approximately $120 per week across the 15-week delivery period.
All work is billed at the lowest service rate offered by the NDIS (currently $73.58ph) and given the broad remit of the program, a range of line items in both CORE & CAPACITY can be used.
Should funding be limited, Roadmap modules can be delivered fortnightly (rather than weekly)
reducing costs to less than $60 per week while maintaining the same program outcomes.
